Post Hospital Care

Once people no longer need hospital care, services to assist in returning home or an interim residential placement can be provided jointly by health and social care services. This helps prevent unnecessary admission to hospital. If you are aged 18 or over, and your needs hold a critical or substantial risk to your independence, you are eligible to apply for independent living assistance. Services to prevent risks increasing may also be available.

How do I get post-hospital care?

If you are still in hospital, then you will need to speak to the key worker or nurse.

If you are at home, then you can contact us directly on (01903 – 755110) and we will arrange a free home visit with you to discuss with you further your needs or you can contact one of the health professionals you are dealing with, such as your G.P. or district nurse.

We are also able to offer a Post Hospital respite service through our strong association with Drumconner who have an excellent reputation for providing high quality nursing care combined with emotional and social support for residents and clients. Every Post hospital respite care package is tailored to your own personal needs and can vary in cost but at all times the most important aspect of the respite is the care being provided to you or your loved one.
